# Acuity Health: Part 7 — Cyber Threat Intelligence & Flare Operations

> Build an actionable CTI program using MITRE ATT&CK, prioritize threat intelligence requirements, and respond dynamically to adversary flare campaigns.

---

Threat intelligence is often misunderstood as a passive collection of IP blocklists and hash feeds. But feeding thousands of unvetted Indicators of Compromise (IoCs) into your firewall does not stop attacks—it creates alert fatigue and blinds your analysts.

True **Cyber Threat Intelligence (CTI)** is about understanding adversary behavior, intent, and capability. It allows security leaders to anticipate which ransomware cartels are targeting healthcare systems, prioritize vulnerability remediation based on active exploitation, and respond decisively during live crisis events.

In this seventh installment of our *Acuity Health Security Architecture* series, we break down how to establish an actionable CTI engine and execute dynamic **Flare Incident Triage**.

---

## The CTI Hierarchy in Healthcare

Effective threat intelligence operates across three distinct operational tiers:

| Intelligence Tier | Primary Audience | Operational Scope & Deliverables |
| :--- | :--- | :--- |
| **1. Strategic Intelligence** | CISO & Executive Board | • Ransomware syndicate market trends and financial exposure<br>• Geopolitical threat climate and regulatory changes<br>• Cyber insurance underwriting compliance posture |
| **2. Operational Intelligence** | Threat Hunters & IR Leads | • Threat actor TTPs mapped to MITRE ATT&CK<br>• Campaign infrastructure tracking and adversary profiling<br>• Defensive posture benchmarking against known attacker playbooks |
| **3. Tactical Intelligence** | SOC Analysts & SIEM Engineers | • Real-time indicators of compromise (IoCs): hashes, C2 IPs, domains<br>• Automated Sentinel detection rule updates and firewall blocks<br>• High-fidelity endpoint telemetry alerting |
| **The Feedback Loop** | Automated Response Pipeline | • Rapid triage and containment validation within 2 hours<br>• Continuous ingestion into SIEM, EDR, and edge WAF rules |

## Mapping Adversary TTPs to MITRE ATT&CK

When tracking advanced threats targeting healthcare, Acuity Health maps threat actor groups (such as FIN12 and BlackCat/ALPHV) against the **MITRE ATT&CK for Enterprise** matrix:

```mermaid
    subgraph InitialAccess ["Initial Access"]
        T1["T1566: Phishing"]
        T2["T1190: Exploit Public API"]
    end
    subgraph Execution ["Execution & Defense Evasion"]
        T3["T1059: PowerShell Scripts"]
        T4["T1562: Impair Defenses"]
    end
    subgraph Lateral ["Lateral Movement"]
        T5["T1021: Remote Services (RDP)"]
        T6["T1550: Use Alternate Auth Material"]
    end
    subgraph Impact ["Exfiltration & Impact"]
        T7["T1567: Exfiltration Over Webhook"]
        T8["T1486: Data Encrypted for Impact"]
    end
    T1 & T2 --> T3 & T4 --> T5 & T6 --> T7 & T8
```

By understanding that healthcare-targeting cartels favor API exploitation (`T1190`) and credential-based lateral movement (`T1550`), the security team prioritizes API gateway authentication context checks and phishing-resistant FIDO2 tokens over generic perimeter firewalls.

## Handling the "Flare": High-Velocity Crisis Triage

In military and incident response terminology, a **Flare** represents an unexpected, urgent spike in adversary activity—such as a zero-day disclosure in an EMR integration or active credential-stuffing campaigns against urgent care portals.

### The 4-Step Flare Triage Protocol:
1. **Assess Credibility & Relevance**: Is the vulnerability or threat campaign applicable to our technology stack? (Filter out noise).
2. **Determine Exposure Window**: Query Microsoft Sentinel telemetry to determine if our public endpoints have observed inbound probes matching the flare.
3. **Deploy Interim Mitigations**: Activate virtual patching rules on the Azure Web Application Firewall (WAF) to block exploit payloads before code patches can be tested.
4. **Disseminate Briefings**: Provide clear operational guidance to clinical desk staff and an executive summary to the CISO within 2 hours.

## Up Next in the Series

In the grand finale of our series, we bring everything together:

In **Part 8: Executive Leadership, Privacy Law & The Capstone Hotwash** (releasing **Tuesday, October 27**), we review our 3-year strategic roadmap, board-level ROI justification, and lessons learned across the entire Acuity Health lifecycle.
